* Who am I, what were my clinical skills problems?

But let us first tell you who am I! Chances are you haven't heard of OSCEhome before. But when you finish reading this, you'll be glad you finally did.

I think I'm a lot like you. I was in the same place you where not too long time ago. I've gone to medical school. . Spend my life going to school, seem like. I've run up these huge debts. I had to move to another city with all the hassle and expenses of settling down and figuring out my way. Medical school was long and expensive. But I've done really really well in everything I tried.

Unfortunately, for the first time of my life I was intern with basically no money. It was a challenge that is really not well defined and, you know, was sort of thrown to the wall and I have to figure out a lot of things. I'll be honest. I had a tremendous fear of failure because all along through school it was clear for me what to do to have success.

But now, all of a sudden I have to worry about all kinds of things, like how to deal with patients, how to take history and do physical, how to be fast and yet thorough, how to make the right decision in a short time, how to deal with nurses and staff. And doing better than my peers in case presentation, exam evaluations, and being a 'good doctor'. Some of these guys, I think, like nothing more than seeing me humiliated in front of them, nurses, staff, and everyone.

There is that whole idea of being out there all of a sudden alone. What would I do? The Hippocratic oath said I'm not supposed to hurt anyone or cause harm, and definitely I don't want to do that. I don't want to miss things or get patient complaints or law suits. So, where to start?

In my patient interviews, I was always nervous and in a hurry. Didn't know the pace I was going. It took me a long time to clarify patient's complaint and HPI. Wasn't able to finish asking all question in one time or forgot the patient's answers, always needed to go back and clarify things and thus my patient notes were deficient.

In my clinical skills evaluation exams, the picture wasn't much different. Adrenaline was high. Patients were of all different kinds. Angry, sad, seductive, talkative and so on. The one I still remember was a patient who kept pacing the room rapidly talking none stop and at the end left the room! Patients who ask stupid questions like scratch marks. Angry patients who takes time to calm them down. I always wished to know a quick way to deal with such difficult patients.

My physical examination was all over the room, not well organized. Takes time, slow patients, and again forgot things to check or looking after the patient's comfort like sliding the bed. I remember once I was so nervous that I used the tooth pick to assess touch sensation which hurt the patient (she moaned). I apologized, but still looked nervous and don't know how to do the test.

My patient notes were not that good too. My hand writing was shaky. HPI not complete and not in sequence as it should be.

As you see, I was basically where you are now when I had all these fears. I had a great success in my scholastic studies in med school. But here, now all of a sudden I was making almost very little money with this crushing debt burden beside all the emotional stresses and I really have to perform. But it was all milky. I didn't know what and how to perform.

When we started our clinical rotation in medical school, we, to some extend, left by our own to learn clinical skills. When we make mistakes, we'll always be able to go back and check with the patient or reexamine. The patient was like a mountain that I have find out our path to climb to reach the dome, that is the diagnosis. I was, and still am, afraid of missing something that will lead to harm the patient or upset him, or make him complain to my attendant affecting my assessment, or even sue me.

Most of the time, patients were supportive. Well, they know I am just a medical student or junior resident trying to learn. Besides, I avoided challenging patients. But this is not the case in real life medical encounters. I haven't enough time to have a relaxed interview pace or even to recheck with the patient. When I was preparing for our Clinical Skills Assessment Medical OSCE Exam, I, just like you, had spent weeks looking for that "Clinical Skills Assessment Medical OSCE Exam magic approach" that could allow us to finally master a thorough and yet time efficient clinical and communication skills approach.

* How did I solved my clinical skills & communication skills problems?

I virtually attended all available expensive Clinical Skills preparation courses, bought and studied almost all available preparation books. I listened to every 'Clinical Skills Assessment Medical OSCE Exam Expert' who offered advice. I spent so much time practicing. But NOTHING WORKED as I hoped! That is to be thorough, organized, and time efficient at the same time and off course good communicators!

Then, four years ago, on a brilliantly sunny day in October, while studying at the central library, I came across few Medical OSCE examiners. They were getting ready for an OSCE exam. I chatted with them and discovered the COLD HARD TRUTH about why I couldn't reach any significant level of confidence in how thorough and quick is my Clinical Skills Assessment Medical OSCE Exam Clinical Skills approach! Most probably, you are now in a similar stressful situation.

The TRUTH about OSCE Exams that I discovered is that my scattered OSCE Exam preparation information, notes, questions, and tips, NEED TO BE gathered in ONE medically thorough, organized, workable, and time efficient step by step SYSTEMATIC APPROACH flowcharts that integrates BOTH clinical skills and communication skills that I can study and practice ONCE for any patient scenario and for ALL my coming OSCE Exams as well as my FUTURE real life medical practice with real patients! An approach that get me high clinical evaluation and scores now, and patient satisfaction avoiding complaints and law suits later.!

Later in life, I discovered that all OSCE Exam stations' formats are essentially the same in all OSCE exams. Doesn't matter whether it is Medical School Students OSCEs, USMLE Step 2 CS, LMCC MCCQE II, PLAB part 2, MRCGP CSA, TRAS 2, AMC Clinical, NZREX, or Clinical Skills Assessment of Foreign / International Medical Graduates. The differences are in the time allocated to the OSCE stations, the actual presence of the OSCE Examiner during the OSCE station, and the depth of needed medical knowledge, that is the OSCE Exam checklists!

They also told me that the OSCE station's stem question is just to direct the OSCE Exam candidate to what system I should focus on giving the limited available time but not meant to omit the differentials and hidden issues. Checklists approach won't get you a high clinical evaluation score!

* So What Did I Do To Avoid Missing Things And Waste The interview's short time?

Over a really long time, I gathered all the necessary information needed to be asked for every disease. Then I:

  • Arranged them in question boxes based on symptom complaints.
  • Wrote/gathered specific questions for each needed info.
  • Checked the medical, ethical, and communication correctness of each question with experts.
  • Determined the differentials for each symptom and merged the necessary questions within the boxes.
  • Arranged the questions within each box in a time efficient flowchart sequence that assures patient cooperation and prevent them from interrupting the medical interview flow.
  • I subdivided each box into a main screening box and detailed sub-boxes to be explored only if the patient reply was related.

So, In a real OSCE, as well as a medical interview, I'll start with the main first question box and follow through expanding into sub-boxes as needed and the back to the main box. Then, according to the station, move to the next box and so on. It took me about a year to perfect the system.

For the physical exam, I:

  • Arranged the needed exams to be done in a time efficient sequence that minimize patient discomfort.
  • Wrote phrases to ask the patient to perform a test or move in a clear, ethical and communication correct manner that prevents patient interruptions to the exam flow.
  • Wrote explaining phrases to say to the patient while performing the steps to keep them informed.

I did the same for counseling stations, patients note, and how to deal with difficult patients.

After going through all these fears, frustration, and the aspiration to figure out this solution. After fighting all that and getting the hard knowledge. I sat down and realized that there is a great need among other medical students, interns, residents, as well as international foreign medical graduates out there to have this information.

That was the approach that I took!... I developed a step by step system. Verified it with my staff physicians. Memorized it. Practiced it. Modified and improved it over and over with the help of my mentors, faculty, and friends. It took OVER ONE YEAR of effort I don't know how many medical students, residents, and graduates helped to develop this advanced system! It was kind of an open project.

Finally, I was satisfied that I had created the best and the most comprehensive system of managing Clinical Skills Assessment Medical OSCE Exam stations and real life medical patient interviews possible and ready to start using it.
